We understand your time is valuable and having an infant keeps you busy.

We want you to know:

• We are seeking parents who had an infant admitted to the NICU after the WHO declared COVID-19 a pandemic (January 30, 2020). 
• This survey should take less than 30 minutes to complete.
• The survey is voluntary and completed anonymously.
• If you are not able to complete the survey all at once, you may return to it at a later time by selecting 'Save and Return later'
• You may choose what questions you wish to answer. 
• You can choose to end your participation and withdraw from the study at any time. Please close the browser and your responses will not be saved.
• While you will not be paid or directly benefit from this study, we believe your participation will add to our knowledge about best practices for supporting parents during NICU hospitalization. 
• All of your responses will be stored confidentially. Only the researchers involved in this study will have access to any information you provide. This survey is anonymous; thus, there will be no way for researchers to identify who you are after you complete the survey.
